For any parent who has watched their child flinch at mealtime injections — or fought to get a reluctant child to take a shot — this week's news is worth pausing on.

On May 29, 2026, the FDA approved Afrezza® (insulin human) Inhalation Powder for use in children and adolescents ages 6 and older living with diabetes. It is the first and only inhaled mealtime insulin approved for pediatric patients.

How Does Inhaled Insulin Work?

Afrezza is delivered using a small, portable inhaler at the beginning of a meal. The insulin is absorbed through the lungs using MannKind's proprietary Technosphere® technology, providing an ultra-rapid onset that more closely mimics the body's natural insulin response to eating.

That speed is actually a significant advantage. Traditional rapid-acting injectable insulin typically takes 15–30 minutes to start working. Afrezza starts acting almost immediately, which can help with post-meal blood sugar spikes and makes it more forgiving when kids eat unpredictably — which is, of course, always.

What the Clinical Trial Showed

The INHALE-1 trial compared children using Afrezza to those on multiple daily injections over 26 weeks. The results showed no statistically significant difference in lung function between the two groups, and no serious lung-related complications were reported.

A few things to know: Afrezza is contraindicated in patients with asthma or chronic obstructive pulmonary disease due to the risk of bronchospasm. Before starting Afrezza, a physical exam and spirometry (lung function test) are required.

Is This Right for Your Child?
