One of the most frightening parts of a type 1 diabetes diagnosis is that it often comes as a sudden emergency. A child gets sick, ends up in the hospital in diabetic ketoacidosis (DKA), and everything changes overnight. For many families, there was no warning at all.

A major new study suggests that it doesn't have to be this way.

The German Screening Study

Over a period of ten years, researchers in Germany screened more than 220,000 children for type 1 diabetes — looking for the autoimmune antibodies that signal T1D is developing, often years before symptoms appear. The findings were striking.

Of the children screened, 590 were found to be in the early stages of the disease. Among the 260 children who eventually progressed to full T1D, 81% had been identified by the screening program ahead of time.

And crucially: families who knew in advance were able to avoid DKA. When a child progresses to stage 3 T1D without any warning, DKA rates can be 25–40%. In screened populations, that number drops dramatically — often to near zero.

Who Is This Relevant To?

Currently, T1D screening in the United States is most commonly offered to siblings and first-degree relatives of people with T1D — because family history increases risk. But the German study screened the general population, regardless of family history, and found that 81% of the children who progressed had no family history of the disease.

This supports a growing argument in the diabetes research community: all children should be screened.

Knowing early doesn't change the genetic risk — but it removes the crisis. And that matters enormously.
